2008 Ordering Information

Here's How to Order Materials

1. Complete the BILL TO: and SHIP TO: addressing information at the top of the Materials Order Summary Sheet.
   

2. Complete individual Materials Order Forms for the materials you wish to order :
   
   Degrees of Reading Power (DRP)
   SSSMART
   Reading Power Essentials (RPE)
   Signposts Early Literacy Assessment System
   MAC II Test of English Language Proficiency
   TextSense Summary Writing
   Educator’s Word Frequency Guide
   Core Knowledge Curriculum-Referenced Tests
   
   

3. Enter totals from each of the individual Materials Order Forms on the Materials Order Summary Sheet.
   

4. Add shipping/handling charges (and sales tax where required).
   

5. If applicable, indicate how and when test materials are to be scored.*
   

6. Sign and date the Materials Order Summary Sheet.
   

7. Attach copies of the Materials Order Summary Sheet and all individual Materials Order Summary Forms to your institutional Purchase Order.
   

8. Individuals and businesses should complete and attach the Registration Form.
   

9. Fax the Materials Order Summary Sheet, all individual Materials Order Forms, the Purchase Order, and the Registration Form (if needed) to Questar at (845) 277-3548
